Balaklava Hospital Service Planning – Community Engagement

The Yorke and Northern Local Health Network has established a Service Planning Steering Group to undertake a service planning process which will identify future needs and service opportunities for the Balaklava Soldiers’ Memorial District Hospital – considering our Hospital, Country Health Connect and Mental Health Services.

An important part of this process is to seek perspectives from the people that use our services and our partners who provide valuable services to our community. We believe that working together to better understand the needs of our community is essential in supporting us to achieve optimal health and wellbeing. We invite you to share your views during the month of May via the following ways:

Attend a drop in “chat session”, pick up a survey and/or contribute to the interactive wall gallery in the foyer at the Balaklava Town Hall on:
o 17 May from 9.30am to 4.30pm
o 18 May from 9.30am to 5.30pm
o 19 May from 9.30am to 1.30pm

Scan the QR code with your camera, click on the drop box at the top of your screen and you will be taken to an online survey. It will take you less than 5 minutes.
 Complete the paper survey available from Wakefield Plains Medical Clinic, the Wakefield Council (Balaklava) or the Balaklava Soldiers’ Memorial District Hospital.
